SOFT TISSUE FILLERS

Soft tissue fillers can be used to rejuvenate areas of the face that have begun to show the signs of age. These fillers are generally used to perform "facial rejuvenation" which would be alternatives to a face lift. Fillers are substances that are injected or implanted to plump up laugh lines, and to fill out wrinkles. Fillers can be used in conjunction with other facial procedures.

PLANNING FOR YOUR TREATMENT OR SURGERY
During your initial consultation, your surgeon will evaluate your face, the skin, muscles and underlying bone. Your doctor will ask about your medical history and discuss any allergies you might have to the products under consideration. Anyone with an auto-immune disease is not a good candidate for these procedures.

You will also be given pre-and postoperative instructions which will include information on eating and drinking and what prescription medications to take and those to avoid. Be sure you and your physician agree on the surgical approach and understand the potential risks and complications involved with the procedure.

THE PROCEDURE
Today, several different types of tissue filler materials are available to provide a younger more rested appearance.

Collagen
Collagen derived from cows is effective on fine and deep lines, contour deformities and some types of scars, including those caused by acne. It’s also used to give lips a fuller, more sensuous look. However, it’s not approved for any other part of the body except the face. When choosing collagen (the type you pick will be determined by what you want to accomplish), you have several options: Zyderm is the lighter form used for superficial lines and usually requires multiple treatments. Zyplast is used for deeper lines and typically requires only one treatment.

Because allergic reactions can occur, it’s necessary to undergo a skin test, usually on the underside of your arm, thirty days prior to collagen injections to determine any sensitivity or allergy. Even with a negative result, there can still be an allergic reaction that can develop immediately, or over time. Collagen’s effects are temporary, lasting anywhere from a few weeks to a year (the average is two to six months). If injections are done over a period of time, there is a risk of scar tissue formation.. Collagen is injected using a fine needle and doesn’t usually require any type of anesthesia. Mild discomfort may be experienced during the injections, and there may be some swelling, bruising and numbness at the sites for the first two days.

Fat Transfer – Autologous Fat
Autologous fat - the body’s own fat - effectively fills deep lines such as the nasolabial folds and plumps up areas like the lips. (It can’t be used on the hands or the breasts.) Also, because the fat is from the patient’s own body, there’s little possibility of rejection. Fat is harvested from a donor site, usually the buttocks or abdomen. The physician gently positions the fat in its location through a tiny incision.

Anesthesia/Level of Discomfort
Fat transfer can be performed under local anesthesia with oral analgesics and sedatives, local anesthesia with intravenous sedation, or with general anesthesia. The type of anesthesia depends on the extent of procedures to be performed, your doctor’s choice and your medical history or desires.

Fat Grafting – Dermal Grafts
Dermal grafts are used to fill deep depressions, such as the nasolabial folds. They can also be placed along the edge of the lips to create an enhanced appearance and may be used in conjunction with other procedures like dermabrasion and laser resurfacing to eliminate acne scars.

Allergic reactions rarely occur because a dermal graft is not a foreign substance. If it’s derived from the patient’s own body, a piece of skin is harvested, usually from the buttocks or groin, and the incision is then sutured. Otherwise, it’s supplied to your physician through a medical supply company. You may experience some swelling at the graft site for about two days. At the graft and donor sites, there may be some bruising and numbness that may last for a few weeks.

Synthetic Materials
A number of synthetic materials are currently being used as soft tissue fillers and the effects may be permanent. They’re effective for deep creases such as the nasolabial folds or furrows between the brows. They can also be placed along the edge of the lips to create a more youthful appearance. What’s more, synthetic materials are easy to revise and/or remove. If misplacement occurs, a revision is simple to do.

RISKS/COMPLICATIONS
Research is ongoing in the development of the perfect soft tissue filler - a filler that’s natural feeling, safe and permanent. New and/or improved soft tissue filler products are being introduced every year. The complications that could occur with collagen include infection, asymmetry, peeling skin and irregularities under the skin.
